Adams–Oliver syndrome (AOS) is an uncommon congenital disorder characterized by a combination of features, including scalp and skull defects known as cutis aplasia congenita, transverse limb defects, and skin mottling. Symptoms of AOS typically involve two primary aspects. First, there is aplasia cutis congenita, which refers to the absence of skin over a specific body area at birth. In the case of AOS, this condition is typically observed at the vertex of the skull. The size of the affected area can vary significantly, ranging from small, hairless patches to extensive exposure of the cranial contents. Secondly, individuals with AOS often present with terminal transverse limb defects. These limb defects can manifest as abnormalities in the upper extremities, lower extremities, or both, and they may include shortened digits or other limb abnormalities.
People with AOS may also exhibit mild growth deficiency, with their height falling below the normal percentile range. Additionally, their skin is often mottled, known as cutis marmorata telangiectatica congenita. In some cases, AOS is associated with other congenital anomalies, such as cardiovascular malformations, cleft lip and palate, abnormal renal (kidney) development, and neurological issues that can manifest as seizure disorders and developmental delays. The syndrome can also involve various vascular abnormalities, including the hypoplastic (underdeveloped) aortic arch, middle cerebral artery abnormalities, pulmonary artery defects, absent portal vein, portal sclerosis, arteriovenous malformations, abnormal umbilical veins, and dilated renal veins. Managing Adams–Oliver syndrome (AOS) primarily involves addressing the specific congenital anomalies in each affected individual. In cases where there are severe defects in the scalp and cranial bones, early surgical intervention with grafting is recommended.
